Scouts BSA Health Forms

Health Form ABC - All scouts and adults attending any event lasting >72 hrs or any event where the participant will be >30 minutes away from EMS services. (Include a photocopy of the front and back of your medical insurance card.)  For scouts and adult leaders, the 30-minute rule applies quite often.  Therefore ALL Troop 50 members, scouts, and adults must use this form.

Who needs to fill out these forms?

Health Forms Parts A, B, and C are required for ALL scouts and adult leaders in Troop 50.

Health Forms Part A and B are used for TEMPORARY participation.  For example, a Webelo scout attends a campout with our troop or a parent chaperones a troop or patrol event (one-time occurrence).

Use this checklist to ensure you are properly filling out and submitting the forms.


Health Form Checklist

        Printed doubled-sided – THREE sheets total for each SET. Make 4 SETS.

o   The 50-60 forms barely fit in the binder – please, no single sided forms!

o   Part B1 on the back of Part A and both pages right side up.

o   Part C on the back of Part B2 and both pages right side up.

Part A

     Scout AND parent signature and date

     Name and phone number of adult(s) allowed to transport the scout

Part B1

     Front & back of Medical Ins. card on ONE sheet of paper (See box on Part B1)

     All fields filled in

     Emergency contact info is legible

 

Part B2

     Yes or No check boxes for Epi-pen and inhalers

     If medications are listed, all fields are filled in

     Yes or No check box for OTC meds

     Parent signature – for OTC meds

     Immunization info is filled in

o   NO extra printouts – parents must transfer the info to the proper form

Part C

     Yes or No checkbox for restrictions

     Healthcare professional fills in: (We had problems with this at summer camp – the scout was not able to participate in anything except lectures)

o   Allergy info

o   Height, Weight, BMI, Blood Pressure, and Pulse

     Healthcare professional checks the boxes for: (We had problems with this at summer camp – the scout was not able to participate in anything except lectures)

o   Normal/Abnormal list

o   True/False list

     Healthcare professional SIGNS and lists the DATE of the EXAM

Copies:

Keep the original AND make FOUR copies.

1. Give a copy to your son in a Ziploc bag. He needs to have it for ALL troop and patrol events.

2. Turn in THREE copies to Becky Cheng. She will check them and then distribute them

Troop 50 Activity Consent Form (Generic)

Generic Activity Consent Form (Parent/Guardian Authorization for Trip/Activity and Medical Treatment Form) - Use this form for all troop and patrol events.

For patrol meetings outside of the church: Ensure that the patrol advisor (or parent of the patrol leader) retains a copy.

Troop 50 Expense/Reimbursement Request Form

Expense/Reimbursement Form - Use this editable spreadsheet to either report expenses or request reimbursement for expenses.  (Clicking either of the two above links gets you your own editable copy but you need to sign into a Google account.)  If you don't have a Google account, clicking on the arrow in the upper right corner below gets you a PDF copy that you can print and handwrite in your info.

The troop will reimburse the Patrol Leader for standard campsite reservations (about $30-35 per site) for patrol outings and for food expenses incurred when hosting one or more Webelos Scouts at a campout.

Troop 50 Service Hours Approval Form

Service Hours Approval - Scouts are HELPFUL!  We believe the reward of service is the service itself.  Scouts should strive to live by the Scout Slogan, "Do a Good Turn Daily." 

However, there are rank requirements where service hours need to be documented.  If you are planning to perform service hours that you want to count towards rank advancement, use this form.  

Be sure to get Scoutmaster approval PRIOR to doing the service if you want it to count for rank advancement.
